For what it's worth, the plaque next to her tomb at Sudeley Castle has it as the 5th: http://tudorhistory.org/castles/sudeley/plaque.jpg I believe that this plaque was copied from the original inscription on her coffin and was copied when her new tomb was made in Victorian times. The Oxford Dictionary of National Biography also lists the 5th as her death date.
It's possible that somewhere it was said that she died on Elizabeth I's birthday (which is the 7th), when I think that was actually the date that Elizabeth received word that she had died. I'm kind of guessing/scratching at cobwebs at the back of my brain ... I vaguely recall reading something along those lines.
